This screening is not about passing a test. It’s about knowing your starting point—and building from there. Before we hit the mountain, ocean, sweat lodge, or breathwork reset, we take stock of your functional baseline so we can keep you safe, customize your plan, and track your growth.
This form helps determine:

Your current ability to squat, carry, walk, float, and move freely

Any red flags around hot/cold exposure, fear, or physical limitations

Modifications that make the CLIMB experience safer and more effective

A roadmap toward trauma-informed movement progression
It’s used alongside your MD/DO medical clearance to create your individualized treatment plan.

Trauma doesn't just live in the mind—it lives in the muscle tension, breath patterns, joint stiffness, and fatigue that shows up in our bodies every day. CLIMB uses:

Ground-based drills (rolling, crawling, barefoot movement)

Water resets (floating, breath holds, immersion)

Hot/cold exposure (sweat lodge, hot springs, ocean)

Agility-specific drills for cops, firefighters, medics, dispatchers, military

Balance, strength, and adaptability challenges

🛠 Who Completes This Form?
This screening can be completed by:

A licensed physical therapist or occupational therapist
-
A certified athletic trainer or personal trainer
-
A physician (MD/DO)
-
A Peer Specialist or Clinical Facilitator under supervision
If you don’t have access to a trainer or therapist, we’ll help coordinate one with your PCP or assign a screener.

You’ll be assigned a Peer Specialist and your movement readiness notes will help build your:
