1. Introduction
Challenger Point Practice PLLC (“we,” “us,” “our”) provides psychiatric diagnostics, clinical and physiological resetting exercises, and facilitates therapeutic experiences (e.g., CLIMB experiences). By submitting this intake form, you (“Client”) agree to these Terms & Conditions, which govern your use of our services and website www.challenger-point.com.
2. Confidentiality & Privacy
All information provided is confidential and protected under HIPAA. We will not sell or share your data, except as required by law (e.g., risk of harm, legal subpoenas, suspected abuse).
3. Not a Substitute for Legal Advice
Content on our website or within the intake form is for informational purposes only. It is not legal advice. Clients facing issues under the Fair Housing Act, discrimination due to Emotional Support Animals (ESA), or Public Service Dogs (PSD) should consult a qualified attorney for guidance.
4. Scope of Services & Required Clearances

Psychiatric Diagnostics & Treatment Planning: We provide clinical evaluations and develop individualized treatment plans.

CLIMB Experiences & Resetting Drills:

Requires a prior MD clearance letter.

Also requires a PT functional assessment with measurable benchmarks and documented warning signs.

Participation is contingent upon these evaluations to ensure safety and appropriateness.
5. Collaborative Treatment Team
Upon meeting prerequisites, you’ll be assigned a Peer Specialist chosen based on your goals and needs—under our clinical supervision and insurance. The full treatment team will consist of:

This team will convene from the outset to establish treatment goals, objectives, roles, and discharge planning.
6. No Emergency or Crisis Services
We do not provide 24/7 crisis or emergency intervention. In any emergency, call 911, go to your nearest emergency department, or dial the Suicide & Crisis Lifeline at 988.
7. Consent to Contact & Communication
By submitting the intake form, you consent to be contacted via phone, email, or secure messaging for scheduling, treatment coordination, and clinical follow‑up.
8. Insurance, Billing & Fees
Submission of this form does not guarantee insurance coverage. You are responsible for:

Verifying benefits, co-pays, deductibles

Any non-covered services
We may assist with insurance verification but cannot guarantee reimbursement.
9. Informed Consent & Separate Agreements
Submission of this intake form is a preliminary step. You must complete and sign separate informed consent forms (clinical services, privacy practices, telehealth, etc.) before any treatment begins.
10. Changes to Terms & Conditions
We may update these Terms & Conditions and will post the latest version on our website with a clear “Last Updated” date. It is your responsibility to review them. Continued use of our services implies acceptance of any changes.
11. Governing Law
These Terms are governed by the laws of the State of Colorado.
